Creemore Tennis Club Waiver of Risk and Liability

By agreeing to purchase or renew membership with the Creemore Tennis Club, you agree to the Waiver of Risk and Liability. Please review the Waiver of Risk and Liability below:

On behalf of myself and junior players in my family group, I hereby release the Creemore Tennis Club and its representatives from any incidents arising from any accidents or injuries howsoever caused from the participation of the applicant(s) of membership during any activities at the Creemore Tennis Club. I understand that all fees are non-refundable. I further agree to abide by the Code of Conduct of the Creemore Tennis Club.
